• 2 pages factsheet – to be printed in colour, double sided printing for one sheet and single side for remaining page, A4.

• 2 pages questionnaire – to be printed in colour, double sided printing for both sheets, A4.

• Word documents and addresses in Excel will be supplied once they are finalised (ready for mail merge).

• Prepaid return envelopes to be used.

All are to be folded and posted using business size envelope.
