Western Health Alliance Limited, trading as Western NSW Primary Health Network (WNSW PHN), is one of the 31 Primary Health Networks across Australia, established to support frontline health services and increase efficiency and effectiveness of primary health care.

WNSW PHN is a not for profit organisation primarily funded by the Australian Government.

WNSW PHN vision is to "Lead, support and strengthen person centered primary health care for our region".

WNSW PHN is seeking a single organisation with appropriate skills and experience to deliver the services for "My Care Future" program.

The My Care Future program is a two part model of Advanced Care Planning. The first part being the employment of coordinators to lead a peer volunteer workforce and the second part being the development and testing of the feasibility model(s) for First Nations communities.

The Request for Proposal provides contextual and technical information for organisations interested in submitting a tender application to undertake the project.
