Northern Territory PHN is inviting RFT applications from suitably qualified organisations and individuals either jointly (in group/consortium arrangements) or independently who can provide services under the Multidisciplinary Teams (MDT) Program in the Alice Springs or Darwin regions.

The RFT seeks to increase access to allied health services to provide person-centred care for Australians that improves health outcomes and reduces avoidable hospitalisations, particularly in relation to chronic disease.

The tender aims to procure the following 3 activities as detailed below:

B. Allied health Services

C. Support Services
