Tender Description

CESPHN invites suitably qualified and experienced providers to deliver GP pharmacist services to smaller and solo general practices for the GP+ Program through a 'Request for Proposal' process. A GP Pharmacist in this RFP refers to a non-dispensing, consultant pharmacist who works in general practice, in collaboration with GPs and will be referred to herein as 'pharmacist'. Details regarding specific activities undertaken by the pharmacist are outlined in the RFP.

The program requires the provision of pharmacist services up to the value of 1.0 FTE, which will initially service 5-10 smaller and solo general practices in the St George region.

Providers that are capable of delivering this service include: community pharmacies, public and private hospitals, private providers and local health districts.

About the RFP

Central and Eastern Sydney PHN (CESPHN) are requesting proposals for pharmacist services for the GP+ Program.

The GP+ Program aims to embed multidisciplinary teams into general practice to support health needs of patients with chronic diseases, especially those from priority population groups. The program is targeted to solo and smaller general practices to encourage greater use of team-based care, increase the use of allied health providers and reduce avoidable hospitalisations. The program will target chronic diseases and health conditions that are amenable to a multidisciplinary approach and support vulnerable populations unable to afford private allied health fee paying or co-payment services.

The GP+ program will involve the commissioning of a pharmacist, which will be a shared resource across participating general practices in the St George region. They will be co-located onsite within general practice, to maximise opportunities to build relationships and establish trust between allied health clinicians, general practitioners and patients.

The pharmacist will deliver non-dispensing, consultant pharmacist services to general practices including the support of management of chronic diseases and complex health conditions, through medication management, reviews and quality improvement activities. Scope of practice may vary slightly between general practices, dependent on local patient needs and demographics, and will be determined through collaboration with GPs prior to commencement of service.
